If you decline the substitution at the pharmacy, you pay the difference compared with the Product of the Month yourself, and it does not count towards the high-cost protection. If your doctor has ruled out substitution for medical reasons, the high-cost protection covers the full price. The amounts are per pack and approximate – the pharmacy calculates the exact amount.
